so the occurrence of others breaking 3T titanium bolts, but having no trouble with other stems doesn't sway you at all, eh.. still seems equally likely to be user error or bad bolt?

Some stems do come with blue threadlocker on the bolts. I installed a Cannondale headshok stem 2 weeks ago that had it, and my 3T Arx stem came with it too.

I am not sure if it's loctite or what,...

You may also be on too small of a frame. 140mm stem is, uh, huge. My first question would be if your top tube is long enough for your torso & arms.
