I had this problem on my girlfriend's bike. There was no way that cog was coming off... we had three people working on it together, with leverage bars and a blowtorch. It was stuck on for good.

The tongue is definitely outrageously large for normal wearing since its an indoor soccer shoe, so I just trim the tongue. There's no padding in it so you can't even tell after you do it.

He just means that the standard, and easier, way is to have the wheel turned towards your front foot. But whatever works is all good. I use both ways depending on the situation if its needed.
