Kuwahara really bumped things up in '87 and '88 with their top of the line Cascade and Shasta being handbuilt on some very good tube...

Probably caused by a little blowout... if you ever wondered what those drilled holes in a frame are for, it is to allow hot gas inside the frame from making an exit through the weld / braze joint.

I suspect the Suntour VGT was a likely candidate... the Huret was also pretty fragile.

Shimano Deerhead was very nice stuff and the friction shifters are among the best ever made.

If you refer to the Arvon frames, they are filet brazed and the only lug is at the bottom bracket and these had to be customized to fit the "mesa oval" tubes which were also custom drawn for these...
