I've seen a wheel where someone was successful getting one on, had to snip it off with dikes.

I've just had one fixed chain drop that I can recall. I dropped the chain on my Free Spirit and luckily it jammed weirdly between chainring, chainstay and tire. Rear wheel locked up, no harm done...
