So much better on that bottom picture. Moved my cleat just a touch in and it relieved the knee pain! No other real pain to think of currently but may need to test it on a longer ride.

It's possible your Ortlieb isn't mounted correctly. The reason I say this is after I bought mine, I was annoyed at the noise as well for a few weeks and decided to change the positioning of the tabs....
