Nope. I’ve worked on recumbent bikes for 40+ years, some of them with miles-long chains. I’ve never had a chain come off, either inside or outside, that didn’t go away after some careful adjustment.

I used several ‘90’s Shimano rear derailleurs...

I used several ‘90’s Shimano rear derailleurs with 34-tooth cogs over the years. They all worked fine. If anything you may have to tweak the B-tension screw.
