It looks mid-70's Japanese, and the fork crown looks to be a copy of a classic, pictured here (not my pic nor my Flickr account): https://www.flickr.com/photos/shu-si...n/photostream/

There are small imperfections around the head tube lugs that make it look repainted to me.
