In europe it is just plain culture, but in developing countries, cars are such luxuries, people just live their whole lives car less.
In my city, in the USA (New Orleans, LA), we have nearly 20% of households with zero cars. Because we are poor, not because we have some philanthropic reasons.

NYC is 50% car free. Because it is expensive and hateful to own one there.
