All KOMs are real men's KOMs IMO. I've never been super fast, and the bay area has a ridiculous number of super fast cyclists. I've probably had around 8 KOMs total. Mainly downhill routes I frequented or lesser traveled segments.
The key to bagging KOMs is to create a bunch of obscure segments. A couple of years ago, we went to visit my aunt in the DR and I took my bike and created a bunch of segments in her area. I'm probably going to have those for awhile.
