Shimano says 45 = 10.5 to 11 US and 45.5 = 11 US.
I think the 45 is closer to a 10.5 than an 11. Make sure you bring cycling socks to test fit, cotton socks felt fine in a 45.5 for me, but with thin cycling socks it was definitely too big.
