If we concentrate on the one joint that failed, one can look at possible causes. A couple of posts down, the idea of a loose fitting post comes up (although I've wondered about it, and did notice the chunk of clamp on the post shows some squishing of the metal.

We don't know about the rest of the frame, and that isn't the issue at this point.
