While on a 30 day heart rhythm monitor, I increased the load until I started having v-tachs, then back off a little. Cardiologist approved of plan and here I am. It took a little adjustment once I started riding on the road, but I'll just watch my heart rate data and ride as much as I can without triggering a v-tach. Life's little curve balls.
Fun fact: Metoprolol Succinate (also known as Toprol-XL) is the ONE extended release medicine I can think of that can be cut or crushed. The actual molecule is metabolized slower. That's unusual with medicines, most "extended release" medicines are coated and can't be crushed/cut yet maintain the extended release property.
