Well, that measurement is "not" including the distance between the pedal and the crank taken up by a bit of axle, so you can add some room for that, so minimum over 4.5 inches clearance. I'd venture to say that most people do not plant their feet "dead center" on the pedal, and likely you don't either when you're actually riding. That size pedal should work great for you, but if not, there are wider ones available. I myself wear shoes that are 5" wide, and those pedals work perfectly for me with no issues whatsoever.

If you really think you need something even wider, these will give you about 5" of room across. If these aren't wide enough, you may want to consider a surgical foot reconfiguration operation. lol
