I strongly endorse bar ends for flat handlebars. I probably use my mountain bike's extensions 80% of the time, because they provide a neutral "handshake" forearm orientation and let me at least approximate the feel of the drops on my road bikes. I have those slick old school 4-finger Shimano brake handles which can be worked surprisingly effectively from the extensions, using one's 4th and 5th fingers.
