I did find a lot of responses on phantom shifting and that likely this phenomena caused by flexing of a steel frame under force of rider going uphill. How old is the bike?
Back in the days of Delrin bodied Simplex Derailleurs we used to get that a lot. The period appropriate solution would be a Suntour VGT derailleur.
