It might be as simple as the RDER cable slipping.
Chain still shouldn't go off the end cog if the High Limit Screw on the RDER is properly set.

Since it's a tricycle, a pic or 2 of the cassette (freewheel?) area may be helpful so one can see just how it's mounted.
