No and no. The stem clamp/spacer stack is first compressed downward with the steerer cap via a bolt to a star nut (for steel) or expansion plug sitting in the steerer tube. Then the stem clamp is tightened for the definitive fix. Nothing can get stuck except, I suppose, an expansion plug, but it's right there an inch away and clearly visible, so easy to dislodge and retrieve.

And is there a specified tightness for that plug? I loosened it before I realized what was what. Yes, this is the first threadless stem I have ever touched
