The rack is a Nitto M18. It has a bar that attaches between the caliper and the front of the fork crown, and the rack bolts to that piece. I adapted a vintage Cannondale bag to use a headset-mounted Velo Orange decaleur. Being a bit shorter than a typical Rando bag, I used longer bolts with spacers to raise the rack about 3cm above the mounting bar. It's proving to be quite stable. Here's a few closer shots, hope that's helpful.
