Plenty of video how to on YouTube. RJ the bike guy is a personal favorite, though not all like him as much as myself.

As mentioned, why not just buy a bike that comes with disc brakes? Bikesdirect has plenty for not too much coin that would be cheaper than trying to convert and modify what you currently have. Cheaper, easier, and will net a better result.
