Cones of front hub loosening weekly
The cones of my front wheel's hubs have been getting loose about every week. I built the wheel with a new hub in 2015, re-packed it a year ago. It rotates freely; as near as I can tell by eyeing it (not professionally) it looks okay. Is this a sign of wear?

If it's coming loose weekly, what damage has been done? maybe worth inspecting the cones/races before doing it up tightly?

It's either an assembly error (not tightening locknut properly) or the bearing surfaces are damaged or deteriorating. Disassembly and inspection of the bearing cones and cups is required to determine which.
I suppose it could also be a broken axle, but that is not too common on front hubs and you likely would have noticed immediately upon inspection, although a QR skewer can somewhat hold an axle together and hide that it is broken.

As mentioned check your locknuts and probably a good idea to undo all the threaded parts and check for any damaged or stripped threads, inner and outer. What make & model are your hubs so we know what type of axle setup you have.
