I eventually discovered that my pain was because my seat post was WAY too high. I had ridden it at that height for quite some time, so I'm not sure why the pain suddenly came on. If you haven't had a professional fitting recently, may be worth rechecking.

There are many causes. I was a very experienced cyclist when I started getting achy balls. It took a while to realize the correlation, because it came a day after I rode a particular bike. I asked my doctor, and he said to change my saddle. The saddle giving the trouble had soft padding and caused no discomfort while riding. But it turned out to be the problem. I now ride very hard saddles. I have no problem whatsoever. My favorite saddles are the Brooks B17 and the Brooks C17. I also like the Specialized Riva which might be out of production.

Sounds like a varicocele. Essentially, varicose veins around the vas deferens. If I remember the numbers correctly, something like 80% of males over the age of 13 have them, but it's only an issue for about 10%. They usually develop during a person's early teens, but don't really become noticeable until later.
Account to my own experience in the wonderful world of swollen veins, there's really not much that can be done about it short of surgery. Keeping your BP down and having the right amount of support helps.

ALL of my testicular discomfort issues were resolved after being diagnosed with Gleason 10 prostate cancer with a bilateral orchiectomy.
