get yourself a bunch of spacers and experiment and use the front mounting part to mount rack to fork. Spacer use is generally ok strength wise, I've used them without issues on a few bikes.
You should be able to find some images of this rack on LHT's online to give you the idea.

Yea you've apparently used the wrong mounting point in the rack. You're supposed to use the front two holes to attach rack to fork and the triangular rear mount is used for fenders. Why is the rear mount used for fenders? Well on top of using the front mounting holes on the rack you're supposed to attach the rack on the rear fork eyelet and that blocks it from fender use.
Of course how it's supposed to be done doesn't really matter all that much and there's room for personal preference. But to get the horizontal bar level usually requires one to attach the rack front mount to the fork rear eyelet
