What cause's rim tape to move?

Just fixed a flat, couldn't find any puncture, check the inner tube. Found a very small hole where it would lay against the inner rim. Checked my rim tape, and there is was, some of the spoke hole showing, probably just enough that it rubbed over time and cause my flat.
Does rim tape shift over time, or is just clumsy tire changing technique? Is this normal?

adhesive velox rim tape shouldn't shift
rubber or nylon band style tapes can slide around when riding with underinflation

I always stay within 5 PSI (120 psi max), so I am a little unsure what happened.
But I am going to pick some velox rim tape when I got to my bike store.

High pressure can cause narrow rim tape to lose an edge inside a spoke hole.
