Bloopers
Since factory quality-control issues are relatively few and far between, this may be a very short thread. That said, the playing field is wide open to interpretation. I bow to your judgement.
Here's a pic of a Super Record headset I acquired in London in the mid-90s. I guess the lettering is type-stamped one character at a time, and the Y in ITALY somehow didn't strike the upper cup during the process:
Doesn't affect its use in the slightest, but it's the only example of a "blooper" I have. What have you got?

^^ Both Campagnolo Nuovo and Super Record headsets spell out "Brev. Campagnolo Italy" twice around the circumference of both top and bottom races. "Italy" is completely spelled out on the other side of the top race and both sides on the bottom. The missing Y is definitely an error.

This one's a real head-scratcher.
Totally Tulular!
Mis-spelled "Tubeing"
Lock Lings!
Rock Rings!
Sharrow of DEATH!
And does this count as a blooper?
Oof! Forgot to glue my tires!

Ah, the ol' rock ring rench.
Saw one on eBay, bought it, came in as a lock ring wrench. Shoulda sent it back.

I have 9 different Billato built bikes right now with the Silva style, hand made pantographed rear brake bridge. The one on the Lemond Gan GLx was mounted in reverse compared to all the rest of them and to all the pics of Billato bikes I've seen on line.

I once brazed on a derailleur cable stop backwards.
Didn't notice it until after powder coat.
Luckily it was my frame, so the owner was ok with me fixing it and using a ballpark matching rattle can fix on it.

English 3 speed newbies (or anyone) don't want to see this when they look for tyres for their club bikes. No bead reference either.

It's difficult to depict the absence of something, but I've examined every conceivable surface of my 197x Raleigh Grand Sports, but there is no serial number on it at all.

Noted in another thread, 1983 Schwinn High Sierra, complete with Giant serial number, made in Taiwan.
But then you have it's head badge, with even about the right date code 1223. There are a couple of others on the forum with the same model bike and the same exact date code. Seems like Schwinn ran out of head badges, so had Giant substitute these instead.
Rescued from the "as-is, needs work pile" at a co-op. Inner chain ring is cratered, missing one of the original wheels, rough paint. But hey, thumb shifters, Deerhead RD, nice saddle, bull moose bars and that head badge, And I did I mention Suntour XC II (removed prior to this picture). what's not to like?

Nice!!
I have a Davidson with the name engraved (pantographed) into the rear dropouts. But when I worked there, I saw another similar frame where the engraving said DAVIDSOV -- they engraver dude apparently quit for the day without finishing the last N, left off that one line nearest to the previous O. Wish I had spotted that dropout before it was brazed into a frame, I would have grabbed that one for my frame. I definitely like the bloopers.

From TTT main.
On my Frejus
By 1963 Ambrosio had introduced the "Gran Prix" stem, which updated the "Champion" by eliminating the point at the rear, behind the edge bolt head (Evian Cycles catalog, 1963, page 43). This was soon updated again, to a recessed allen-key wedge bolt. Some of these stems included the text "MADE IN JTALY" (sic) on the right side, and "GRAN PRIX" on the left.

Makes one wonder if they were outsourcing stems and forgot to change letters completely before going back to stamping them as locally-produced.
