It may just be luck, but, while I have had persistent coughing bouts a few times, I've not had an sick-in-bed illess in five or so years.

I would definitely tell them that they are going to pedal strike and go down. I would also get them behind me so I don't get collected in the crash.

Current preferred MTB technique Is to keep your pedals at 3 and 9 through all curves. That might simplify things for that guy.
Or if he really wants to drop a foot maybe take him skiing and he should be able to figure out which foot to drop after a day on the slopes.
