I'm not so sure about that. I've seen plenty of actual road bikes with more than 30mm worth of spacers. I say road bikes because they have far less of a tendency to be set up jackassedly. Check these bikes out, they're from the "post your bike" thread over at the road forums. And for reference, most stems have between 35-45mm clamp height.

I've heard that more spacers will have an affect on the stiffness of the steerer tube that you may be able to feel in a heavy sprint or steep, standing climb, but never heard anything about it being a safety issue.

Though I agree, 60mm+ spacers is still silly.

All but one of those stems seem to be tilted down.

There are a few reasons why I suspect that those roadies have their stems pointed down with lots of spacers:
1) Pointed downward is more pro-looking
2) Steerer tube extended over the stem is less custom-looking
3) They want to keep a decent amount of steerer tube for resale value as once it's cut, it's gone forever and may be too low for the potential buyer.

I think that's pretty silly. If you want the power from your arms to make it through to the cranks (and ultimately the rear wheel) then you want as few spacers as possible. The spacers will compress more than the stem. Also up high you are also torquing higher up on the steerer tube which gives you more leverage to flex it. If the bars need to be higher, then flip the stem up and remove 1/2 the spacers.

This is the look they are going for. A fully custom bike will have no use for spacers above or below the stem. Downward tilted stem. This is my ride:
