I had my bicuspid aortic valve replaced with a porcine valve 8 years ago. Is your brother a cyclist, runner, etc.? One thing I wish that I spent more time on was deep breathing. My breath-hold/lung capacity is nowhere near pre-surgery levels (this is very noticeable when I go freediving/spearfishing). He could be on a stationary bike now spinning taking it easy.

brother is not a cyclist. interesting that your performance declined after surgery. or am I misunderstanding?

I'm not sure if my cycling performance has declined due to the surgery. I wasn't racing for a few years before surgery and the local fast group rides' courses coincidentally changed dramatically while I recovered/rehabbed so nothing resembling metrics are available. My breath hold decline was very dramatic. Before surgery, I could drop down 20 feet or so and hang for a while looking for fish. After surgery, I would drop down and then need to come right back up. (They did have to open me up 3 times within 7 weeks and that might have something to do with it.)

I have been the proud owner of a St. Jude aortic valve prosthesis since August of 1990.

It sounds silly, but tell your brother to "Enjoy the adventure." I was a strange experience to need a rest half way up a hill on my neighborhood walk -- The same hill I once ran up pushing my twins in a stroller. It is best to get off the pain meds asap without being a hero. And also to exercise and get off the other meds that interfere with exercising. If he is not involved in some form of endurance athletics, encourage him to begin and use one of the apps like wahoo where he can track his progrress with distance and speed at a given heart rate. Where ever he is at now, improvement is guaranteed and interesting to monitor.
