1. 120 miles, but that was 30 years ago. In the past decade, somewhere in the low 80s.
2. 25 miles into the hills.
3. 15 miles, if kitting up means spandex and cleats. However, I do commute in MTB shorts, old Avocet touring shoes, gloves, helmet, t-shirt and cycling jacket as needed. The commute is only 4 miles each way, but it's nearly all uphill from the flats to the top of the Berkeley Hills (~700 ft). I sweat a lot, so I need a change of work clothes.

Minimum? MTB maybe 15 but that is a big depends on where. not MTB bike in the 30's. I don't really kit up, I put on clothes appropriate for the ride/weather, it's not traditional bike kit.
I know my miles per year but when looking for my average distance over time I noticed for the last three years my miles were about the same and I am gradually riding further per ride but riding less times. Avg distance was 24 in 2017 and 33.4 in 2018.
The longest I will travel by car to ride is 1x my total ride time (1/2 each way). I recently moved and the opposite of where my typical routes are so that may change but on the positive side, my routine workout route now involves no driving and I start in my driveway.

I don't really kit out for any ride, don't have a jersey big enough to fit, only wear shorts if I'm heading out for more than 2 hours. Will wear leg warmers, sweatpants or cold weather tights weather dependent. All but one of my bikes have a platform or platform/clipless pedal, only the racing bike has just clipless. So even shoes are optional.

Or if you're talking in a day, then I've done a couple 24-hour Time Trials ... 400-something km.
2. Average? I've been cycling avidly since 1990 and occasionally before that. I've ridden recreationally. I've raced. I've commuted. I've toured. And I've been involved in Audax/Randonneuring since 2001. My ride lengths have been all over the place.
My husband (Rowan) had a very serious workplace accident in March 2018 which resulted in a severe traumatic brain injury. So the rides are short these days.
3. As part of Rowan's recovery process we try to get out for rides now and then. They aren't long and perhaps the shortest ones are about 10 km.

In my long, long distance days, it was usually about 75 km before I had really settled in and felt comfortable.

Bike shorts - depends on time of year. Sweating in underwear is no bueno.
I'm lazy enough to ride (vs. walk) if it's more than half a mile. No kit for that, though.

Shortest willing........midweek local night rides 10 miles to recover and stretch the legs.
Longest 126 miles with 7,200 ft of elevation gain
Most demanding.......Breathless Agony. 114 miles with 12,000 ft of elevation gain all in the first 72 miles.

14 miles usual commute (can be extended occasionally to 20 to 30 miles when training for a century; weekend training rides about 40 to 70 miles)

For me, that’s a ride of minimum of ten miles, and my commute is a minimum of14 miles. I only warm up, and then ride at cruising pace to the end.
When I first started a training schedule for a century, I noted at what mile I spontaneously realized I felt completely warmed up, i.e. I did not consciously look for it, and it usually occurred at about 6 miles.

...My basic training is to ride at my RPE [Relative Perceived Exertion] of 50% for six miles to warm up, then cruise at an RPE of 60%, and do intervals (on hills) at 70%:
- [70% is described as“hard”;
- 60% is“somewhat hard,"
- 80% is “very hard" (lactate threshold;breakpoint between hard but steady breathing and labored with gasping).
- 50% is "fairly light" (my usual happy-go-lucky pace without thinking about it)]

I rode centuries when I was young, but since returning to riding in my 50s my longest ride has been about 65 miles.
Normal rides are about 30 miles.
But I'll get out in the evenings for 8 to 10 miles at times.if time is limited.

Don't have special "kit" clothing, so I don't worry about much other than keeping dry and being seen, so it's pretty easy to toss on a jacket, helmet, reflective vest and go.
Legs don't have the "oomph" they once did, due to old injuries. 30mi max is about all I can do.
