Alright so other then Lake and 'EE' size shoes does anyone know any manufacturers that make a road shoe with a wide toe box?

The higher end Specialized shoes might be a little more form fitting. I have a pair of the Sport Road and there's plenty of room for my big feet in there, but if you're in S-Works shoes I doubt you'll want that much of a downgrade.

I'm using Sidi Genius 5 Mega shoes and they're not bad. Used some Shimanos for 10 or so years. They weren't bad. But I guess after a couple of hours my feet must swell significantly because they are my first discomfort point. Long before my ass or hands.
I have been thinking for years I should probably get some custom shoes but I've never got around to A) justifying the money and B) having the patience to wait for delivery.

Not wide in the toe box...I have a set of Expert Road shoes. Toe box isn't wide at all for my needs. I tolerate them for now until they wear out, then will be looking for something else.
The only option with custom widths for specialized is the $350 SWorks line.

My feet are 9 4E width which makes it tough to find the right shoes. I used to wear Sidi Megas but when they changed the shape of the replaceable heel pad they also changed the rear of the shoe and made it wider. I switched to Northwave shoes which have wide toe boxes but the rear is still narrow enough for my feet. Not a bling shoe but I've used them on several double centuries successfully.

Try the Bontrager RXXXL. I actually had to go down a half a size (to a 44.5) and there's a ton of room in the toe box—most definitely the roomiest toe box shoe I've ever tried on! I had a pair Specialized Pros (45.5), which I wore for 3 years and thought they were ok in the toe box. But the RXXXLs are really roomy! The only problem with them is that they're extremely vented so you probably don't want to take them for a ride in the winter without a good bootie to cover them up.
I live in Vegas, so I'm pretty excited to wear the RXXXLs once the temps start to climb, but I'm looking at the Mavic Zxelliums for cooler temps, since the mesh is supposedly thick and deceiving based on one of the on-line reviews. The Mavics tend to run a little narrow, but I'm finding that going up a half a size on them really helps. I had tried on an 11 in both the Zxelliums and the Pro-Road. My toes felt a little cramped. I ordered 11.5 in both shoes from an online site and just got the Pro-Roads today. The 11.5 (46) fits great! I can't wait to get the Zxelliums and try those, too! The most comfortable shoe wins! The Pro-Roads are really comfortable, but I'm betting the Zxelliums will be even more comfortable based on how they felt when I tried on the 11's.

Longing for a Tail Wind
I was shopping last year and found some Bontrager and the Specialized BG shoes were ok in terms of a roomy toe box.
Then I found some Sidi Megas on sale and went with them. They are a bit hard to fit though, so be careful with sizing.
The Bontragers were pretty good. But it could depend on the model.

I have a EE foot and Shimano R-087W shoes fit me just fine. I've worn them on rides in excess of 4 hours without any increase in tightness in the toe box. If my feet swell, and they probably do, the toe box is wide enough to accommodate it.
I have also heard good things about Diadora shoes in this respect.

My feet are only through the ball of my feet and toes. The sidi mega size is perfect. I cant wear most wide shoes, because they are too wide throughout the entire shoe, and the heel cup is much too wide. Sidi's fit perfect.

Lake. I have somewhat wide feet and have a couple of the regular ones which feel roomy enough. They also have wide models which must be a veritable hotel room in there though I haven't tried any.

I wear 4E New Balance running shoes and I can fit fine into Specialized BG Comps. S-Works comes in a wide size. I had and returned a Sidi Mega. It was not that Mega

The wide S-works model only goes up to size 46 I think. And the regular 46 is so short (toes pressing) that I passed. Am going to try the regular S-Works shoe in a 47 at LBS this weekend. If that fits, I may go for the "74 model. Nice combo of leather and BOA. I've had SIDI mega Genius in the past - nice shoes.

I went with a pair of Sidi Ergo 2's that conformed to my feet.
But the Bontragers were noticeably wider from the beginning.
I think they'd be worth a look along with Sidi Mega Wides.
