Close. General internal medicine. The value based programs I have seen are generally well intentioned but not necessarily all that helpful.

We’ve had a lot of success changing physician behavior in orthopedics, specifically over utilization of inpatient rehab and skilled nursing. Got one practice from sending 50% of their lower joint replacements to SNF down to 5, their readmission rates are also half of what they once were

I believe that. The programs we have been involved with seem to focus on us coding better, which I get but still is gaming....along with trying to save the whole system money (ACO), last year we saved Medicare a few million bucks but apparently not quite enough to trigger getting a share of the savings.

Actually took a road bike for a ride. I'd forgotten how nice my old Mercian rides. And how much thinking ahead you have to do with DT shifters and toe clips and straps. especially with slotted cleats. On the plus side a NR rear derailleur makes a nice satisfying chunk when you change gear. I'm going to need to replace my gloves though: worn gloves and thin, cloth covered bars is not a good combination.
