The cotter pins have obviously loosened off, as the crankarms are about 15 degrees out of alignment with each other.

I wouldn't notice something like big feet, as they're the norm in our household. My wife wears size 12 and my son wears size 15. On short chain stay bicycles, his heel can contact the rear derailleur when in the smaller cogs and cause it to jump out of gear. As a result, his current bicycle is hybrid, with sufficiently long chain stays.

Also, that there is daylight appearing in the middle of the mountainside?
She's unapologetically appearing as a model, so the high heels seem appropriate. Had she been presumed to be a rider, one might anticipate a wardrobe malfunction of the sort that, who knows, might be deliberate (recalling such from last year on the bike trail through Folsom).
