I think we are halfway to the ideal setup to allow good fender mounting, good reflector mounting, and proper brake function...

Starting from the brake bridge and moving along the brake bolt I think you should have...

[brake bridge]-[convex washer*]-[reflector bracket]-[thin washer]-[nut]-[thin washer]-[fender bracket]-[thin washer]-[nut]

*a convex washer, similar to the one between the brake and the brake bridge, will allow you to get maximum clamping force around the brake bridge. A bike co-op or bike shop will likely have a convex washer if you can't find one.
