Replace BB bearings or new cartridge BB
Not spinning smoothly (noisy too)
BB needs work (not smooth). Is replace BB with cartridge the way to go?

Sure, if only the balls are the problem. Which is doubtful. Generally the axle/cone contact/rolling surfaces get worn/pitted first then the larger surface of the cups next. I have pulled BBs (and hubs or other bike bearings) where the balls were still shinny and even in condition but the cup or cones are trashed. But why speculate? Take the thing apart and you won't have to ask others on the condition, you'll see it yourself. Andy

If the current bottom bracket axle and/or cups are badly pitted, rusted or otherwise damaged, replacing it with a cartridge is the easiest way to proceed. You can probably still find replacement parts for the old cup-and-cone bb but it will take some searching to find the right ones and, unless you have some affection for this type, cartridges are cheap, easier to install and very durable.
But, as Andy recommended, disassemble what you have as see what's wrong.
