When I first started riding seriously at 50, I needed more arch in my insoles or my feet hurt, so I used special cycling insoles, don't remember the brand. I don't need them in my cycling shoes anymore - I think that either my foot muscles have gotten stronger or I'm using better cycling shoes. I still use arched insoles in my hiking shoes, Superfeet Green. Probably more arch than I need, but they're fine. My wife has custom orthotics in her hiking shoes, but doesn't need them in her bike shoes. Sidi Dominators for us both.

My wife's right ankle "bends" in toward the crank. I have installed extenders. She doesn't complain but as I ride behind her I notice it. Insert won't work because she wears Shimano cycling sandals.

If she's happy, you're supposed to be happy, right? Or else talk her into trying shoes.

I don't see why an insert of some sort could not be glued to the sandal. My favorite glue is 3M 5200 fast cure. When it is cured, it is amazingly tenacious. Before gluing in permanently, I'd stick the insert on with double stick tape to get the location right.
