About 13 years ago on a flat road with my rigid MTB with V brakes. I was going about 20 MPH when 2 big dogs ran out of the woods onto the road right in front of me. I reflexively hit the brakes hard. The front wheel stopped, but the rest of the bike and I didn't. 2 badly bruised hands, 2 fractured wrists, one fractured kneecap and one shattered elbow. The bike was pretty messed up too. The dogs were fine.

So if I hit a concrete retaining wall going downhill on a Schwinn Typhoon wilst trying to use the coaster brake, go over the bars and land on the front lawn, does that count? Shame that I don't have video. I think I got max style points. Did it twice, without a helmet.

A group of pedestrians turned unexpectedly and walked out in front of me. Got the bike stopped but with the rear wheel way up in the air before falling over sideways. Bike was a Schwinn Varsity with Weinmann sidepull brakes.

As a kid I once endoed while putting on only the front brakes while simultaneously dismounting. I never endoed again in my life (so I put never). Hard for me to imagine how one could since I never had tens of thousands of miles of different kind of riding on different kinds of brakes (now hydraulic disc).

Was riding a friends MTB on the road and braked...granted, a little firmly...expecting to skid to a stop. Instead, i learned several valuable lessons:
1. Vbrakes can lock the wheel hard compared to the anemic road rim calipers I was familiar with.
2. Knobby, wide tires don’t necessarily skid on rough asphalt.
3. My friend, a motorcyclist, had reversed the brake levers (right lever to front brake).
Endo’d just before a 4-way stop sign, in plain sight of cars in all directions...must have been quite a spectacle. I tore back skin on both forearms near the elbow, and a little on my right knee...not bad, but had to get some stitches. Biggest wound by far was to my ego

Nope, not braking. I've come close a time or two, but never over the bars.
I've gone over the bars when I should've braked, instead of doing a number of stupid things not considered braking.

As a paperboy several decades ago, heading downhill on soft ground with a Wald basket filled to overflowing I learned the folly of grabbing a handful of front brake. Instantaneous and impressive weight transfer.

I would like to add that I have had scary issues with canti's that were weak enough to be dangerous than ever having had any issue with the most powerful hydraulic brakes.

No. Not while braking. When I was 9 or 10 I did a jump ramp on my Schwinn Stingray, the 60s/70s kind. About the 10th jump I landed front wheel 1st and went through the high rise bars.

I was gonna go for 'Just about everyday' but that only really applies to the 1980s. Except for the first couple of years, as my Raleigh Grifter was too heavy to lift the rear wheel.
So i went for 'Several times'. Because, several times on a BMX in 1987, several times on a Falcon Sierra in 1988, several times on a Diamond Bac....

I've been over twice when I used to commute. Once when someone opened their passenger side door, and once when someone drove around me and turned left without any indication. Both in heavy traffic. No damage in the first and forearms and knee abrasions in the second which also took out the front forks.
Both were the better option than the alternative, lol!

Almost. Some kid stepped out in front of me on an MUP. I was only going about 10 mph. Slammed on the brakes and nearly went over. Threw my weight back and stopped myself from doing so.

First time, was 40 years ago. Raleigh Record road bike, center pull caliper brakes. Traveling at low speed on a gravel shoulder, upset about recent events, I grabbed both brakes hard in anger. Back wheel came up in what seemed like slow motion, almost balanced at the top, and I then toppled forward. I tucked my head and rolled on my shoulder. No helmet and no injuries.
Second time was 20+ years ago. Wife called attention to a turtle in distress so I looked to the side, not realizing that wife hit the brakes. When I looked forward there she was at a stop. I hit the brakes hard, went over fast, again doing the shoulder roll. Again, no injury, but when the rear wheel came around it hit the pavement quite hard, knocking it out of round. I had to wrestle it back to near enough round with hands and feet, just to ride home with the cantilever brake disconnected.
The good news is the turtle was ok!

The closest I've come is a time when my front tire had a puncture and gotten low on air. I took a corner too fast and the tire folded up. It sent me sideways more than an endo.

Last autumn, my front brake lever hit a falling leaf, locked up the wheel and I went over. The dangers of hydro are real.

As a kid I'd put up roofing sheets on a few cinder blocks to jump going down a little hill beside our house. Once the blocks shot out right as I hit the peak of the ramp. Superman onto my chest with lots of scrapes. Surprised I didn't break a collarbone. Maybe I did, dunno. It hurt a lot.

But hydraulic disc brakes on road bikes will cause it. In fact, grab a handful of front disc and you'll go over before you can even break your fall with your hands . Not only that, those disc brakes are MUCH stronger than rim. And they'll blow your front fork.

Many years ago I decided to adjust my front V brake cable without knowing what the hell I was doing. Pulled it in about a 1/2 inch. Rode down the street and grabbed it hard as I had done before. Of course the neighbors and all of their kids saw the event.

Yes. I was fixing my daughter's new-used bike and took if for a test ride. I pulled each brake lever separately and when I did the front I ended up on the pavement. Small bike/tall person so a higher center of gravity in relation to the center of the wheel, slight downhill, complete stop of the front tire and low enough speed not to skid yet fast enough my body didn't want to stop, and I was on the ground. Likely a interesting physics 101 problem to give kids.
Maybe not true endo'd in the mountain bike purist sense, but I know for a fact my ass was higher then my head. I was able to tuck and roll to avoid head damage, and the bike was behind me after landing.
I had road rash on my elbow and it hurt. The next morning I couldn't move it much, so I got an x-ray. During the x-ray they forced my arm straight and the elbow stopped shooting sharp pains and loosened up. So something was pinched in there.
Call it whatever. All I remember is looking at a front wheel completely stopped by the brakes and significant rotation while in the air. So I know the cause and the affect.
I probably wouldn't have won money with a video of it but might have gotten on Ridiculousness.
