My wife brought up yesterday that we have routine dental check ups on the 25th and wondering if they'll be cancelled. This morning's local news answered that question as officials asked that non-serious dental appointments should be cancelled. Agreed.

I just told our medical director I feel strongly that we should fast-track the telemedicine process. Many of our patients can be evaluated by face-time or other methods and we can discuss their symptoms, their health, their blood test results, even look at that rash, without them coming to the office. I think the technology exists and the ability to get paid for it exists, but putting the whole thing together is going to take some time.

One other big frustration for me is the utter inability to have my patients tested for COVID-19. I am told that my office does NOT have plans to do testing. We have a sister office a couple miles away and they have an outdoor tent set up for testing, but I am told for now we can't send my patients there. And the local health department apparently has no plans to offer testing either.

I haven't either, at least locally.

I did have to run to the local grocery store to pick up some coffee yesterday, and took a stroll through all the aisles just to see what people were stocking up on. The usual things like cleaning supplies, TP/paper towels, canned foods, and water were all gone, but what surprised me the most was the significant lack of pancake syrup. Not actual maple syrup, mind you, but the good ol' 'murican HFCS stuff

I’m at my second health appointment/test this week and the security is rapidly changing. Today, I was escorted by a security guard to await my test in a holding area. Yesterday, there was no escort and the waiting room had 10x the number of people as today. Evidently, a lot of appointments were cancelled by the hospital and/or patients in the last 24 hours.

I saw 13 patients yesterday and, as of now, will be seeing 10 today.

That's half my usual volume.

I already have communicated with about 10 patients today through our portal (it's primitive email-like, but secure) and addressed acute/chronic issues, reviewed tests, sent in medicines etc., which I think is great but that is something we (as of now) don't get paid for. My hope is that changes asap. People are saying that it will.

We had interesting conversation in our emergency doctor meeting yesterday. I suggested deferring most elective orthopedic procedures like hip replacements and repair of torn cruciate ligaments. I was surprised to get pushback from the orthopods. That stuff is their bread and butter.

But three seconds prior in the meeting they were all in on the idea of moving to essential services.

I think it’s human nature to believe the one thing they personally want to do is “ok” whereas everyone else’s thing is the problem.

Thats why I think the specifics have to come from above. Closing public beaches during Spring Break when you know folks are in your town specifically to go to the beach for spring break is probably necessary. Otherwise people think- “well I’m going outdoors which is ok and I plan to stay 6 feet away from folks I haven’t been exposed to, and I’m heading home in 3 days when it’s more practical to start real social distancing, so going to the beach today is ok”.
