Are there serrations or some kind of knurling on the center area of the bars?
What type of metal are the parts made out of?
I have a feeling that as an adult reliving his childhood, you're pushing the parts far beyond their design specs.
A steel stem might just grip the bars along the edges of a clamp, while an aluminum stem may grip the whole width of the clamp.

I had the same problem. Someone suggested puting a piece or two of aluminum foil as a shim. I crinkled it first, then flattened. It worked well.
