I understood your OP. I didn't answer the question about chain rings, because I think any orthopedic benefits of the Biospace chainrings are minimal, at best, if any at all. I used them once in 30-years of cycling...I wasn't impressed.

Your orthopedist performed an X-ray on your knees and didn't see anything, yet you still have pain.

Thanks for resubmitting! I must have overlooked it and, indeed, that's what my mindset has been since relocating in 2016. I'm gonna get back to it...

Gotta say that a fitting session may not provide the answer you are looking for. As it is with all fits, the adjustments come slowly. I did fits for many years and found that not all the adjustments transferred equally to the road. Make sure the fitter provides more than a return visit or two, but a time period after the fit for fine tuning adjustments such as saddle height, reach, hood placement, etc. 6 months is reasonable. With full fits we provided a 1 year period. Most did not take advantage of this feature, some did with good cause, and others did it because it was free and figured they paid for it.

Just get a longer racket for your pickleball games!!! Then you won't have to bend so much.

Small Aid For Knee Pain

Since it is January, it thought I would mention that excessive cold on knees is not good. Be sure to cover your knees with bib knickers, bib tights, knee warmers, leg warmers etc. I know this is not your main problem but every little bit helps

True. When I dealt with this issue years ago, it was almost always during the wintertime...

Are you riding clipless pedals? My knee and foot pain went away when I started riding platform pedals, even on group rides. I am not any slower and I have compared. Recently I went to Pedaling Innovations Catalyst pedals which are huge and you pedal mid foot, so the ball and heel are involved. They are tremendous for me and I know my friends will give me a hard time, but the benefit is huge for me.
i received an email from Pedaling Innovations where a doctor had wrote he felt the big pedal mid-foot riding was easier on knees too.
make whatever adjustments you need to keep riding.

Up the cadence, reduce the pedal pressure to get the same power out the cranks which reduces pressure on the joints. Make sure seat height is correct - seat posts do tend to slip down and it's good to check periodically.

And, FWIW and to help confuse things, I believe Chris Froome uses elliptical chain rings.

I had knee pain from lots of tennis playing in my 20s. Ending up needing surgery to clean up problems with a torn meniscus and scar tissue. Maybe you should see a specialist, make sure you know what's going on. For me it felt like bailing wire tightening inside my knee, and it came and went.

I ride biopace and regular cranks. Really, hardly any difference in terms of knee strain or rotational forces.

As discussed various ways above, I think spending money on a pro fit might be a good idea for you once you are sure you are not dealing with some structural thing that needs surgery.

I've passed the magic Medicare date, and I am still running 4 times are week - year 4; more on asphalt than on dirt. So. knee problems are not necessarily a permanent disability. You just need a good diagnosis and some smart workout tactics once you figure out what's going on.

I have a history of bad knees from playing basketball and handball in my youth. Cycling never did bother them.

My hybrid has oval chain rings, yet I notice no difference spinning it and my road bike.

That said, I too recently took up pickleball and played a bit too much in an effort to quickly get better. My knees began to suffer, so I simply alternated days of cycling and pickleball. Works for me.
