In the absence of 9probably unobtainable) data, I wouldn't hazard a guess as to how big the problem may be, but I have no doubt that bad setup causes some knee problems in some people with clipless pedals.

Wow. You seem to be saying that you can't conceive of bad clip setup causing or exacerbating a knee problem. If I read you right, THAT is excessive skepticism. Personally, I would guess that bad seat height causes more knee problems than clip setup does, but I can guarantee that I would have had knee problems if I had set up my clips the way the pedal salesman told to. (I use pinned flat pedals, and I can reproduce the problem any time I place my feet in the position the guy recommended.)

I'm saying just because you or I can conceive it, it doesn't make it true. As I said, I am extremely skeptical that someone would continue in such an unsuitable and uncomfortable setup long enough to do real damage. You yourself just gave an example of how someone (namely you) spotted the problem right away. Do you really think other people are too stupid to also figure it out?
I can conceive of riding on a seat post without a seat installed leading to internal bleeding, pretty sure that doesn't happen either.

I did the heal-to-pedal method for years; however, after having a pro bike fit I learned that my saddle was about 1/2" too high. That, coupled with a slight tweak to my cleat angles, and the back pain I suffered with for years magically disappeared

Just my experience but that method tends to put the saddle too high which causes knee pain. Get a goniometer. Your angle should be about 30 degrees at full extension. When I do this I notice I’m always within a mm or two of Lemond’s chart.

That creaking feeling is called crepitus. Usually it suggests arthritic changes in your knees. I too think a trip to an orthopedic Dr and some x-rays to rule it out are in order first thing. Then if they find nothing (fingers crossed) you have to remember none of us are built exactly the same, and what works for one doesn't work for the other.Play with the position until you find what works for YOU.
My knees are toast, as in severe arthritis in both, but if I set my seat a bit higher I can ride without pain. And its actually made my knees stronger and less apt to pop out of joint or fail because the tendons and ligaments are strong.
And oddly the knee issues seem to be something that runs in the family. Mom had them, all of my brothers have had knee surgery at some point. And a grand niece had to have surgery to straighten out the leg or it would have destroyed her knees to keep playing sports. She got it taken care of before it caused damage, With knees that is the trick, the earlier you catch it the better your outcome. Don't do what I did and slog through until there is little they can do but replace the knee joint.

I had just the opposite experience - I had chronic knee pain in my early riding years, until I got clipless pedals, then I haven't had any pain since. But I've always used SPD pedals, so maybe their greater float has something to do with this.

Even though I have my bike set up right with the Heel to Pedal method, it hurts my left knee after a ride. Like my left knee is shattering. I also can feel creaking in both knees when I lay my palms on my knees and moving my knees in a swinging motion. I place my left knee on the floor while stopping and starting.
If you are using clipless pedals, you might not have them adjusted correctly. When I first started using them, my knees got pain. After I did some adjustments, then I didn't have pain in knees anymore. I was using spd type so I can adjust the position of the feet.

Knee Issue
Had Knee issues for years. Went to see the doc and he recommended a special exercise. He had me put my left knee in and pull my left knee out and then put my left knee in and shake it all about. Believe me it was fun and it worked!
