Installed new, wider wheels which required a full brake adjustment with new pads. Did the entire adjustment and a 10 mile test ride and only upon putting the bike away did I realize I did the rear brake adjustment with the caliper quick release open.

Got to the event with the bike on the roof rack only to discover that the key to the roof rack was on the other key ring, still at home.

How about more than one mistake with the same thing?
Went mtn biking with a buddy, drove to the trailhead, and in pulling the bike off the fork-mount rack I twisted the handlebars, breaking a dropout in half.
Years later,
I installed a crown race on a new fork by bracing it on the concrete floor, crushing the dropouts.

Similar to the OP,
on a long, steep & brutal mtn bike climb- a fire road but washed out so quite technical-
I red-lined it almost all the way to the top & realized that I had been in the middle ring.
