Notices
Road Cycling “It is by riding a bicycle that you learn the contours of a country best, since you have to sweat up the hills and coast down them. Thus you remember them as they actually are, while in a motor car only a high hill impresses you, and you have no such accurate remembrance of country you have driven through as you gain by riding a bicycle.” -- Ernest Hemingway

I don't know what TTP is but his PCV is 10% today. IM is thinking he probably has immune-mediated hemolytic anemia. Abdominal ultrasound was neg.

He's too weak to really assess his neuro status. He could have something like chronic disc disease that looks 10x worse because of the severe anemia. Once the anemia is resolved, we can get a better neuro exam.

Decided to use my lunch break to hit up the grocery store. I really wanted to get some basic meat for the week, but as I understand it they are restocking every night only to be picked over by the evening. I didn't want to deal with the hassle of a busy Saturday morning run.

Pickings were slim, but there was chicken and beef. Even snagged some ground pork.

This is also an odd time to find out that I must buy the least popular bread. Shelves were nearly empty but there were still many bags of our preferred brand. Funny.

Thrombotic thrombocytopenic purpura. Notorious among human neurologists for producing strange, multifocal, vascular syndromes, along with jaundice, splenomegaly, etc. and falls into that immune hemolytic anemia category.

Another day with stressed out staff and patients. I am converting to telemedicine. For now I am going to try to see as few patients IRL as possible. I did 8 virtual visits today and the patients seemed to like it ok.

Rumor has it that the state will increase current restrictions to “shelter in place” except for essential activities. I hope that includes riding.

Sounds very difficult. At least you’re not telemedicine-ing with dogs.

My brother, the Seattle ER doc and internet medicine tech startup guy, is stressed to the limit. His wife runs a hospice nursing service and both of them are flat out scared of dying.

I feel very lucky and safe compared to both of you.

Tough times.

Yes. People don’t seem to care about fresh stuff so other than potatoes, veggies were plentiful. Not a single sweet potato or those wrapped potatoes for microwaving. All kinds of bulk russet potatoes, though.

Frozen veggies were slim pickings except for broccoli which I like so I picked up a couple bags.

Before anyone asks, I didn’t look for TP, wipes, or paper towels but I’m guessing there weren’t any as I didn’t see anyone checking out with any.

Dogs: not febrile, dramatically anemic, mildly thrombocytopenic, maybe a little azotemia but not overt symptomatic renal failue, sometimes various vascular events including in the nervous system but not super-commonly

PA governor has just ordered that all non-life-sustaining businesses must close by 8 tonight. Wonder where that leaves me.

Interesting times, to say the least. Up here (Ontario, CA), we are going through the same thing.

At present, 'emergency powers' have decreed that all restaurants/bars be closed; exception: places can be open for pick-up/take out.

Most 'non-essential' businesses are closed or closing: shopping malls/clothing stores and so on. Shortages are of course commonplace. Managed to find two cases of paper towels today, and a couple bottles of 70% alcohol to use for surface sterilization. No TP to be had today; I will be up and out on a run tomorrow a.m. to try to find some.

I work (faculty/admin) at a major university. Typical weekday population is around 40,000. The students are gone; 'non-essential' staff are gone. Essentially, maintenance staff remain on site, and faculty offices remain accessible for the duration. Research labs are running, but no new projects can be initiated. That's it. We have moved our courses 'online' for the duration of the academic year. Place looks like something from a sci-fi film: lots of birds/geese, and nothing much else.

The masterminds in Washington have proposed a stimulus package where everyone, except Democrats I assume, gets $1000 and I also read $1200. I wish they would speed it up as Spring is nearly upon us and I could use some bike gear.

NPR reported it was going to be $1000 for each adult and $500 per child. Then it would repeat again mid May (I think)... I’d think there’d be an economic threshold to qualify for it, but let the handwringing commence.

I second this. Interesting when I told the wife about a ritchey breakaway that was 4000, she said okay. But, when she says I can use the 1000 for a new bike, and I mention the emonda and domane are around 2400 she says hell no. I missed something.

I bet it finds its way into the trump foundation

one general tip for when the exact hub cone is not readily available. discovered that the curvature of the bearing surface on the shimano 600 was a pretty good "skeleton key" for many applications. it would sometimes require a small change in ball size to make everything come out correctly.

Current Senate Republican proposal phases out completely at family income of 99,000/yr. Reasonable number, I guess.

So I was talking to our criticalist today. I was thinking if push comes to shove, should we offer up our ventilator for human use?

Apparently I’m not the first one to think of that. She showed me a huge spreadsheet put together by the Critical Care college of veterinary-owned ventilators that will be offered up for human use should the need arise. All the contact details, where the ventilators are and so on. It will be a drop in the bucket. But each one of those could save some lives.
