What am I seeing here? Was this a diamond frame with the top tube cut and relocated to make it a step through? If so, why is there a lug on the seat tube? What happened to this?

Creative indeed. I am amazed that it has a lug on the seat tube in the new location. Amazing that someone went to the trouble to put a lug in, but it doesn't look good with the top tube bent and the original seat lug unfinished.

Umhum... that's not a girls bike it's what is generally referred to nowadays as a sloped top tube.

Perhaps there was an accessory attachment that went from the front of the existing top tube to the lug at the top of the seat tube. It was there to make it look like a boy's bike. You take it off, and voilà, you have a girl's bike. I've seen a few like that.
