Here we go ...

What I'm assuming is an ST-6700 Flight Deck 10-speed right shifter with the cable head snapped off within the shifter. The ratchet mechanism will click twice from the tightest (lowest gear, innermost position) before it no longer responds to the small paddle.

So, what's the first step?

Remove it from the handlebar? Do I attempt to begin disassembling the shifter? Is there a technical note on this from Shimano?

The bike belongs to the wife of a friend, and I'd prefer not to replace the shifter...

Suggestions? I'm all ears...and thank you.

Pull the hood up from the bottom and see if you are lucky enough to be able to fish it out. Check shimano's tech doc site for the DM, but probably won't be there for that model. Newer models are similar and will give you some ideas.

Top of hood only lets you remove the lever. Though I'm not sure about your older model.

Same thing happened to me last fall. I cut the hanging bit off, pulled the hood back.

Then I looked at it a bit.
Maybe had a beer?

I seem to remember that it was a pain to index all the way over but it could be done.

Idk. I got it out and it's good now.

It's a 2008 Madone 5.1, for the record.

What I need to do is to expose the rotating cam/ratcheting plastic part that spools the cable. It's jammed between 3 and 4, so I must rotate it back to 10 to gain access to the cable leaded end.

Since the cable is broken/ jammed do you need to gently tug in it to get it to move in the right direction. Maybe I did that.

I recall that when I finished it was one of those "that wasn't so bad" moments.

I had one snap off a few years ago. I took the bars off so I had something to hold on to, squirted some Tri-flow (I think) in and just kept working it back and forth. Did it on the couch watching tv for about an hour one evening, then again the next. All the sudden it let loose and rotated down all the way. Hint: don't get tri-flow on the couch.

Can you see the cable on the spool? If so, you have a fighting chance. I picked up a free pair of those shifters that had a broken cable with the ball end wedged in the bottom of the housing. It took me along time (days) to finally figure out how to get a tiny forceps into the housing to pull the cable out. In the end, I did it and have a nice set of shifters on my gravel bike. If the cable is still on the spool, you might be able to work it a bit with some small tools (forceps, probes, needle-nose tweezers). I’ve attached a pic of how far I had to take the shifter apart before I could get to the cable end. Good luck!

Dripped into the interior...minutes of working the paddles, a probe, tweezers and voila!!!
