The guy who filled out the form for me (I was in no condition to do that. I was actually told I could sign with an X.) quoted me. They were my words.

I signed, but you can’t really make out any letters. Maybe the first letter of my first name. There was also a bed-side notary.

My advice is to make sure you have your paperwork in order while you’re healthy. Make the decisions so your peeps don’t have to. I’ll be executing my will on Friday.

I don't know why Gene C. didn't occur to me. I'd often wondered what his name signified (and usually my brain just modified it to "genetic" for some reason). RIP.
