It looks like an alloy TA other ring has been mounted to a cottered steel crank as a cuff guard? Odd thing to do.

No, it is a standard half-step plus granny. 42-39-26. I usually use 42-38-26 but I had an old 39 in my stash and figured it was close enough. On these old bikes I don’t ride very fast so the low gearing is all I need.

Nice set up. Looks like an old frame with 32mm tires as well. Clever. This is what makes bicycle mechanics fun. The older stuff seems to lend itself to these types of tinkering better (i think) than the new stuff. I don't work on the new stuff too often so perhaps I am off base here.
