I like my DT shifters. Work great, cheap, light, ultra reliable. Everything I want in a commuter. Or a long-distance bike. On my racy bike, I have brifters...but I'm regularly dropped by a guy...

Well there is another fairly obvious possibility. Since the problem happened with the same stem, from the same manufacturer, speccing the same bolts....maybe the bolts really are faulty. It's not...

Come on guys. He used a bicycle specific tool, designed specifically to avoid breaking lightweight components and fasteners on bicycles.
