Also depends on where your IRL riding is. Some of the virtual world/routes have gradients not found in some locales, so trainer difficulty setting can help I suppose in emulating your own geography.

Sorta, but reducing trainer difficulty reduces the amount of shifting you need to do. Also, potentially with a steep enough gradient, if your trainer bike doesn't have low enough gearing, at 100%...
