There is more to it than that, but yeah, the healthcare system is dysfunctional in many ways.
I understand and get it, and also agree with you.
The fee issue is the reason I will not work for insurance companies. They have commoditized their work to the point where they are dictating that lawyers are paid very low rates or low flat rates per case, and I want nothing to do with that. A lot of the insurance defense lawyers are getting paid $100-150/hour and being told what they can and cannot do in the course of a case. No thank you.
