Back when I used 7 speed stuff I remember having a roller fall off and just the pin was there. This happened more than once.
I had that a lot back in the 80s but I rode through snow, rain, etc. and ran the chains until they were as loose as could be. My LBS blamed it on salt water at the time because the rollers would be galled and then would crack.
