It depends on the Garmin and the type of file it wants or will accept. For crazy reasons, ridewithGPS will NOT allow me to export a .FIT file without a paid subscription. My Garmin 130 only accepts a .FIT file so that was a problem. It will allow you to export a .GPX and other formats with the FREE version. So like most questions with technology, it gets complicated in a hurry.

Do you even need to export them? I'm certain that, with the Wahoo, at least, I never had to - I just had to sync my RwGPS account with my Wahoo account and any routes created or pinned would migrate over automagically.

Have you ever looked at what your billing people are submitting? There are modifier codes etc that can increase the payment in a pretty meaningful way for the more in depth visits.

Maybe. I know there is a "Garmin Connect" app that can sync routes, but that is such a piece of crap I typically just download the routes to my PC and then drag/drop them into my Garmin which I have plugged in via the USB. That's how I have done it but I expect there are other ways. I borrowed a Hammerhead Karoo and it worked simply by syncing as you describe.

We have to do our own billing and my understanding is that the vast majority of third party payors reimburse MD/DO the same as NP/PA. There are a few that reimburse them at 85% but I think that is when they are alone and not working "under" a physician. The whole thing is a dysfunctional scam IMHO.

Maybe the billing is wrong? I heard a podcast recently on how the biller/insurance person is one of the most important people at a healthcare provider.

Anything is possible but pretty sure my billing is right. Most of my visits are straightforward office visits, level 3 or 4. Occasionally I bundle a physical exam with an office visit and use a -25 modifier, but the main Blue Cross/Blue Shield insurer does not like that and cuts the payment for the office visit by 50% when we do that. We have no choice but to suck it up. Or go Gault.

Strange, here in NY I’m 99.99% positive that they have to be working under the supervision of a physician by law.

One of the primary reasons I never go to urgent care is that they’re typically staffed with NPs and PAs, I’d rather get a same day sick visit with my PCP, I get better care and it costs me less out of pocket.

Does your practice participate in any full risk models? I’ve seen family practice physicians making specialty surgery money under those models. It’s tough to get going and requires a pretty heavy up front investment in case management, but usually pays dividends. Who’s the big payer in your area? Highmark?

We have some risk contracts and ACO patients. The big payer here is IBC and they actually pay us less than Medicare, last time I checked. Plus the 50% cut if we use -25 modifier.

Less than Medicare is insane. I’d love to know how they get away with that when most payers are banging out 125-150% of Medicare for Part C or Commercial members

Okay, I just poked around - yeah, if, from within your RwGPS account, you connect/authorize your Garmin Connect account, the routes created/pinned will migrate over to your Garmin head unit without jumping through any other hoops (other than making sure that your devices are talking to each other and you have internet access).

Can you just not bundle?

I have no idea how medical billing works.

It's occurred to me that both your kids are now in school. Do you tend to do your vacuuming at the same time everyday? Floors? Laundry? Kitchen cleanup? Daytime soaps and gameshows?
